Injury Waiver

In consideration of acceptance of this registration by the club, I HEREBY RELEASE, WAIVE AND FOREVER DISCHARGE the club and each of its members, officers and employees FROM ANY AND ALL claims, demands, damages, costs, expenses, actions and cause of action, whether in law or equity in respect of death, injury, loss or damage to myself or property, arising or to arise by reason of my participation in the club excluding any negligent act, whether by omission or commission of any of the aforesaid persons, parties or entities.

Membership Selection Instructions

Team Registration Instructions
Please enter the names of all team members. Each team member must submit their own registration form.
If you would like to play but are not currently part of a team, enter your name in your preferred playing position and indicate “TBD” for the remaining team members.

Tuesday Evening Mixed League
For the Tuesday evening mixed league, you may register as a single, double, treble, or full team. Teams may have no more than two male players.

Please ensure that:

You enter your name in the position you wish to play.
If you are registering with teammates, enter their names in their respective playing positions.
Enter “TBD” for any positions that have not yet been filled.

Additional Information

2nd-year curler: This refers to someone in their second year of curling overall, not their second year at SPCC.
Preferred playing position: In the “Notes for League Manager” section, please indicate your preferred playing position.
