Injury Waiver

In consideration of acceptance of this registration by the club, I HEREBY RELEASE, WAIVE AND FOREVER DISCHARGE the club and each of its members, officers and employees FROM ANY AND ALL claims, demands, damages, costs, expenses, actions and cause of action, whether in law or equity in respect of death, injury, loss or damage to myself or property, arising or to arise by reason of my participation in the club excluding any negligent act, whether by omission or commission of any of the aforesaid persons, parties or entities.

Membership Selection Instructions

Please enter all team member names for 'choose your own' leagues, each team member must submit their own form. If you wish to play in this league and are currently not a member of a team, put your name in the position you would like to play and indicate 'TBD' for the other team members.

Please note that for the Tuesday evening mixed format you can sign up as a single, double, treble or full team with no more than 2 males on a team. Make sure to input your name in the position you are playing and if you are joining with other teammates, also indicate their names in their respective positions. Make sure to input TBD for any positions that are not filled.
